- Inpatient Insurance In accordance with the terms and conditions previously stated that the claim process is carried out by completing the claim form and all required documents a maximum of 30 calendar days from the date of filing the claim. Documents needed:
- Photocopy of the Insured identity card / KTP and Family Card;
- Medical resumes that have been signed by a doctor who treats and/or seals the hospital or a photocopy of a medical resume;
- Copies of legalized receipts from the hospital with full details of treatment costs;
Legalized copies of Inpatient evidence from the Hospital.
- Life insurance The insurance claim process is carried out by making written notification of the claim must be submitted to the Insurer within no later than 90 calendar days from the time the Insured dies by attaching the following documents:
- Death Claim Form issued by the Insurer that has been filled out completely and correctly;
- Death certificate from an authorized government agency;
- Certificate of causes of death from doctors issued by the Hospital (there must be an explanation of the diagnosis);
- Certificate from the Police in the case of unnatural death or accident;
- A statement from the local Indonesian Embassy (KBRI) in the case of his death abroad;
- Photocopy of valid proof of identity of the Insured and the designated party (heir), in the form of a Family Card and KTP / SIM / Passport / KITAS / Certificate of Domicile from the kelurahan of the Insured.
